Office Policies

Appointments

Appointments can be made by calling our office during normal business hours.

Cancellation Policy

We require a 24 hour notice when cancelling or rescheduling appointments. No Shows for appointments are subject to a fee of $25.00, procedure visits are subject to a fee of $50.00 and $100.00 no show fee for hospital based surgeries. As a courtesy to our patients we do call the day before to remind them of the appointment.

After Hours & Emergency Care

If you are experiencing an emergency, dial 911 or head to the nearest emergency room. For all other calls, an on-call physician is available 24-hours a day, 7-days a week.

Prescription Renewals

Prescriptions are renewed during normal office hours with a 48 hour turnaround. In some instances, we may require that the patient be seen in our office prior to the medication renewal.

Financial

All office visits are payable at the time of service. We accept cash, checks, and most major credit cards and debit cards. Please contact our billing office should you have any financial questions or concerns.

Referrals

If your insurance company requires a referral for your visit, please be aware that it is your responsibility to obtain the referral.

Forms Fee

Due to the number of forms that insurance companies require for prior authorizations, disability, life insurance applications and employers requiring FMLA paperwork we must charge a fee of $15.00 for each set of forms needed for these requests.
